A good part of the opposition to these assertions of mine may well be explained by the fact that the sexuality from which I derive the psychoneurotic symptoms has been equated with the normal sexual instinct. But psychoanalysis teaches us more. It shows that the symptoms by no means arise solely at the expense of the so-called normal sexual instinct (at least not exclusively or predominantly), but represent the converted expression of instincts that would be described as perverse (in the broadest sense) if, without being diverted from consciousness, they could express themselves directly in purposes of fantasy and in actions. The symptoms are thus formed in part at the expense of abnormal sexuality. Neurosis is, so to speak, the negative of perversion (30).
The sexual instinct of psychoneurotics exhibits all the aberrations that we have studied as variations of the normal and as manifestations of pathological sexual life.
a) In all neurotics (without exception), impulses of inversion, a fixation of the libido upon persons of the same sex, are found in unconscious mental life. Without a deeply penetrating discussion it is impossible to appreciate adequately the significance of this factor for the formation of the clinical picture. I can only affirm that the unconscious inclination to inversion is never absent and is especially valuable in elucidating male hysteria (31).
b) In psychoneurotics, all inclinations toward anatomical transgressions can be demonstrated in the unconscious and as producers of symptoms. Among them, with particular frequency and intensity, are those that claim for the mucous membranes of the mouth and anus the role of genitals.
c) An especially prominent role among the producers of symptoms in the psychoneuroses is played by the component instincts, which usually occur in pairs of opposites and which we have come to know as bringing new sexual aims: the instinct for looking and exhibition, and the instinct for cruelty developed in active and passive forms. The contribution of the latter is indispensable to understanding the painful nature of the symptoms and almost regularly governs a portion of the patients' social behavior. Through this linkage of the libido with cruelty there also takes place the transformation of love into hatred, and of tender into hostile impulses, which is characteristic of a great number of neurotic cases and indeed, as it appears, of paranoia as a whole.
Interest in these findings is further increased by several peculiarities of the facts.
